Avulsion fracture of the anterior tibial tuberosity in pediatric patients: a series of clinical cases and a review of the literature

ABSTRACT

Introduction: avulsion fractures of the tibial tuberosity are uncommon injuries representing 0.4- 2.7% of all physeal fractures, mainly affecting adolescent males. They are often associated with obesity and sportsrelated trauma. Timely diagnosis and management are essential to restore extensor mechanism function and prevent complications. Material and methods: we report a case series of four pediatric patients treated at the Hospital General de Cancún between 2024 and 2025. Clinical data including injury mechanism, classification (Watson- Jones modified by Ogden), treatment, and outcomes were collected. Results: three cases were classified as type IIIA and one as type IIA. All underwent open reduction and internal fixation with cannulated screws and washers; two cases required additional patellar tendon repair. Postoperative outcomes were favorable, with bone healing and functional recovery. One patient experienced temporary range-of-motion limitation. Conclusions: tibial tuberosity avulsion fractures require accurate diagnosis and classification. Surgical treatment yields good outcomes in displaced type II and III fractures. Early rehabilitation contributes to optimal functional recovery.
