Epidemiological overview of pathological terrain fractures associated with multiple myeloma in a tertiary care hospital from 2019 to 2024

ABSTRACT

Introduction: multiple myeloma is a neoplastic disease of native B cells, characterized by the growth and accumulation of B lymphocytes and plasma cells in the bone marrow and, less frequently, in other extramedullary organs. It is the second most prevalent hematological disease, 2.12 × 100,000 people. Objective: to determine the epidemiological panorama of fractures in pathological terrain associated with multiple myeloma in the population of the General Ignacio Zaragoza Regional Hospital in the period from 2019 to 2024. Material and methods: a cross-sectional, observational, retrospective study was conducted by extracting data from the clinical records consulted, including diagnosis, fracture incidence, comorbidities, and functional capacity. Results: the most common pathological fracture location was the spine, with 46.7% (n = 28), and secondly, the femur, 28.3%. A relationship was found between the presence of comorbidities and the Karnofsky Functional Score (χ2 = 7.75, p = 0.00), and chronic kidney disease was also associated with the score (χ2 = 15.58, p = 0.03). Conclusion: the most common location was the spine, with the thoracic region being the most affected. The presence of comorbidities, specifically chronic kidney disease, negatively affects functionality, which is consistent with what has been described in the international literature.
