COVID-19 and corticosteroids as a risk factors for avascular necrosis of the hip: a retrospective study at Angeles Mocel Hospital

ABSTRACT
Introduction: avascular necrosis (AVN) of the hip is a
degenerative condition resulting from alterations in the
intraosseous blood supply. Since the COVID-19 pandemic
and the widespread use of corticosteroids, an increase in
AVN cases has been observed, even among patients without
previous risk factors. Proposed mechanisms include systemic
inflammation, vasculitis, and hypercoagulability. This situation
has raised concern regarding the relationship between steroid
therapy and the development of AVN, making it necessary to
study this association in the Mexican population.
Material
and methods: between January 2020 and June 2025, 11
patients with hip AVN were studied at Hospital Angeles Mocel.
Results: a total of 81.8% had a history of COVID-19, and 72.7%
received corticosteroids. Unilateral involvement predominated,
mainly grade II–III, with an average symptom onset of 12
months post-COVID; 54.5% underwent total hip arthroplasty
and 27.3% underwent core decompression.
Conclusion: a
significant association (p‹ 0.028) was found between a history
of COVID-19 and the development of hip AVN in patients treated
with corticosteroids. Clinical and radiological follow-up during
the first year post-infection is recommended.
