ABSTRACT

Synovial anterior cruciate ligament (ACL) cysts are infrequent findings, usually asymptomatic. We present the case of an adolescent athlete with anterior knee pain without a traumatic history, in whom magnetic resonance imaging (MRI) showed cystic lesions adjacent to the ACL, with characteristics of pure liquid content. MRI allowed differentiation from other entities, such as mucoid degeneration or partial rupture. This case highlights the usefulness of MRI for the non-invasive diagnosis of cystic ACL lesions in young athletes, avoiding unnecessary interventions and allowing conservative management. Its pathophysiology and differential diagnosis are discussed.
