Chilaiditi’s syndrome as a surgical decision problem. report of one case and review of the literature

ABSTRACT
Introduction: The hepatodiaphragmatic interposition of the intestine is a rare abnormality in the general population. It was described as syndrome in 1911 by Chilaiditi, reserving the term of Chilaiditi’s syndrome for those cases accompanied by clinical symptoms.Objective: To present the case of an old man with intestinal occlusion, in whom the Chilaiditi’s syndrome could be integrated.
Case presentation: A 85-year-old man with pleural pain, dyspepsia, nausea, vomiting, abdominal distension and constipation. The physical examination revealed slight abdominal distension, painful palpation of the right superior quadrant and tympanites. Laboratory results revealed slight leukocytosis and serum sodium of 128 mmol/l; X-rays showed air in the right hypochondrium without signs of bowel obstruction. The patient was managed with conservative therapy attaining complete recovery of the intestinal transit.
Conclusions: The Chilaiditi’s syndrome is a rare entity that can be identified with simple radiographic methods.
