Laparoscopic surgery of the peptic ulcer perforation. New concepts

ABSTRACT
Introduction: Peptic ulcer is a common condition that may lead to a duodenal or gastric perforation. Nowadays Helicobacter pylori is known as the etiologic factor for most of these ulcers and recurrent ulcers after Helicobacter erradication are very unusual, thus we can effectively solve the surgical problem of these patients with just a stich on the perforated ulcer plus abdominal lavage. This technique can be performed laparoscopically with good results.
We wanted to find out which one was the approach chosen by surgeons who managed these patients in our province during the year 2002.
Material and methods: Retrospective multicentric study of all peptic ulcer perforation cases in the province of Gerona during the year 2002.
Results: Thirty five patients were submitted to surgery for peptic ulcer perforations, average age was 53.3 ± 18.6 and 57.1% were men. Twenty six (74.3%) were operated by laparotomy meanwhile 9 (25.7%) by laparoscopic approach. Overall average length of hospital stay was 8.1 ± 3.6 days. There were no mortality or complications in the laparoscopic group.
Conclusions: In the province of Gerona during the year 2002, 25.7% of patients suffering from peptic ulcer perforations were operated by laparoscopic approach. This approach was safe and effective.
