Long esophageal myotomy by laparoscopy as a treatment for esophagus diffuse spasm. Results and technique description

ABSTRACT

Objectives: To show the experience of long esophageal myotomy by laparoscopy as a treatment for esophagus diffuse spasm. Seat: Third level private hospitals, Mexico City. Design: Prospective, longitudinal, non-randomized, non-comparative, multicentered study. Material and methods: Nine patients underwent long esophageal myotomy by laparoscopy between October of 1993 and October of 2003. The following factors were analyzed: age, sex, medical background, pre and postoperative auxiliary diagnostic studies, surgical time, hospital stay, time of restoration to habitual activities, the surgical procedure morbidity-mortality, and symptomatology postoperative relief. Results: Five of the patients were female and 4 male, with an average age of 48.7 years old. The surgical time presented an average time of 3 hours. The postoperative esophageal transit with hydrosoluble contrast was normal, the patients tolerated a liquid diet during the firsts 24 hours and were discharge after 72 hours. The average period to return to their normal activities was of 9 days. No complications and mortality were reported. After a follow-up period of 10 years, the patients were free of symptoms. Conclusions: The results from ten years using the described surgical technique of long esophageal myotomy by laparoscopy is an effective and safe method that presents all the benefits of the least-invasive surgery.
