Infección congénita por citomegalovirus. Pautas para su diagnóstico

ABSTRACT

Objective: To report the clinical experience in the diagnosis of congenital infection of cytomegalovirus.
Material and method: Clinical records of 11 patients with clinical suspicion of this infection, were review.
Results: None had cytomegalovirus suspicion. Main findings were: microcephaly (8/11), spastic motor deficit (9/11), convulsions (8/11), hipoacusia (4/9). All seric IgG titles were elevated and IgM was positive in two patients; one of them died from active disease (pneumonitis). Cranial computed scan showed periventricular calcifications in 2/11, cortico-subcortical atrophy in 9/11, colpocephaly 1/11 and left frontal porencephalic cyst in 1/11. Three magnetic resonance image were performed, two were normal and the other one showed bilateral hyperintensities in the white substance in T2, and in the basal ganglia.
Conclusions: Frequency of this infection in our hospitalized population had a 2.5%. Since the disease was not suspected in the referal units, did not send patients on time to a third level unit. Suspicion signs are microcephaly, hypotony, and convulsions.
Subependimal calcifications are not always demonstrated in the computed tomography scan. Serological scrutinity should be useful in identifying asymptomatic patients with irreversible sequela risk.
