Pulmonary coccidioidomycosis: 24 years of experience in the children hospital of Sonora

ABSTRACT
Objective. To describe the epidemiological and clinical features of pulmonary coccidioidomycosis (PC) in the children of Sonora State.
Material and methods. 64 clinical files of children with PC were founded in between 1983-2006 and information was obtained about the clinical features, diagnosis, laboratory findings and the medical treatment.
Results. In 24 years the incidence rate of PC was 5.6 cases in 10,000 admissions. The rage of age from 2 to 18 years and most of the children have 1-4 years age. The clinical findings were: cough (82.8%), fever (76.5%), lost of weight (26.5%), cervical adenopathy (31.2%), exanthema (17.2%). Radiological features included reticulogranular infiltrate (31.8%), mediastinum widened (25%) and pleural effusion (16.6%). Coccidioidin skin test was positive in 65.5%. Titers of IgM were positive in 62.2% and IgG in 73.7%. Histopathological identification was made in 12.5%. Culture was positive in 6%, and fungi identification was done by gastric aspiration/pleural effusion and/or bronchial brushing in 28.5% of the cases.
