Diagnosis of parotid gland tumors: clinical characteristics and fine needle aspiration biopsy

ABSTRACT
Objective: To evaluate the clinical characteristics of patients with benign and malignant neoplasms of the parotid gland to know whether, through them, benign and malignant disease can be differentiated; besides, evaluating the diagnostic capability of FNAB in these tumors.Patient and methods:Retrospective, transversal study comparing patients with parotid gland tumors. Assessed variables were: age, gender, presence of cancer, histopathological diagnosis, time of evolution, tumor size, presence of symptomatology consisting of pain, facial nerve paralysis, dysphagia. Diagnostic value of FNAB was also assessed. Statistical analysis was performed by means of Student’s t and chi square tests; sensitivity, specificity, positive and negative predictive values, as well as positive and negative odds ratio of FNAB were also determined.
Results:One hundred and five patients were studied, 53 women (50.5%) and 52 men (49.5%); 76 benign cases (72.4%) and 29 malignant (27.6%). The only clinical characteristics that yielded a statistical difference in regard to benign or malignant diseases were age and size of the tumor, being low in the benign cases. FNAB had a sensitivity of 63.1%, specificity of 90.6%, positive predictive value of 75% and negative predictive value of 84.7%, accuracy of 82.2%. Positive odds ratio of 0.40 and negative odds ratio of 6.71.
Conclusion:The signs and symptoms reported in the literature, representing the clinical manifestations of an advanced malignant neoplasm are not as frequent, and do not allow for a clinical differentiation between a benign and a malignant neoplasm, hence the surgeon has to assess any parotid growth with FNAB, which depicts a good diagnostic value.
