Myringoplastya with cutaneous allograft acellular (Alloderm): new alternative for the handling of the tympanic perforations

ABSTRACT

A cross-sectional and descriptive prospectivo study was made to compare and to prove the effectiveness of alloderm and fascia temporary, as material of graft for closing of simple timpánicas perforations, of infectious or traumatic etiología. 40 patients of the external consultation of Otorrinolaringología of the Military Central Hospital with central tympanic perforation, dry ear by but of 60 days and integrity of the chain studied to osicular. 20 patients were dealt with cutaneous allograft acellular (ACA) (Alloderm) under local anesthesia and with endomeatal technique, placing two layers of graft (to under-over) and a support of gelfoam in average ear. The 20 remaining patients handled themselves by means of homograft of fascia temporary (HFT) (to under-over). In the group of patients of (HFT) a total closing in 16 patients was obtained (80%), in the group handled with (ACA) the total closing in 17 patients (85%). The aerial-bony difference in the audiometría was of less than 10 dB. In most of the patients. The use of (ACA) for the repair of tympanic perforations is useful and presents/displays the following advantages, with respect to fascia temporary: it reduces the surgical time, it eliminates the morbidity of the donating site and can be made like ambulatory procedure, even in the doctor’s office. We observed a case of probable rejection and extrusion of the graft in one of the patients operated with (ACA) whom antecedent to the material of absorbible suture had by ricochet. In all the cases of residual perforations important diminution of the perforation surface was obtained. The use of (ACA) is a useful alternative to replace the homograft and its application showed the same effectiveness that fascia temporary.
