Management at primary hyperparathyroidism due to parathyroid adenoma at a regional hospital in Spain

ABSTRACT

The purpose of this paper is to report our experience at a non-specialized Spanish regional hospital with the use of minimally invasive surgery for the treatment of primary hyperparathyroidism. Three cases were evaluated and treated, mean age was 45 years, whereas mean serum Calcium, serum PTH and 24-h urine calcium were 11.4 mg/dL, 168 pg/mL and 405 mg; none of the cases showed bone involvement. The day of the surgery, patients received a 5 mCi dose of Tc99m at the Nuclear Medicine Department of our reference tertiary care hospital. They were then immediately transferred to our hospital were the operation took place, using the «Europrobe» to localize radioactive tissue. In the 3 cases, the affected was thus correctly localized and excised, confirming its nature by the ex vivo continuous emission of radioactivity. The success of the procedure was also confirmed by the normalization of serum calcium and by the histopathological finding of a parathyroid adenoma. This report, anecdotal as it may be demonstrates the plausibility of the minimally invasive parathyroidectomy at a non-specialized hospital, provided there is support and coordination by a good Nuclear Medicine facility.
