Intramyocardial injection «sowing» technique for cellular therapy in terminal cardiac insufficiency. Original procedure

ABSTRACT

Introduction: Within the surgical procedures for treatment of terminal heart failure, cell transplantation can be used individually or in combination with surgical procedures such as myocardial revascularization. Implantation techniques, however, have given rise to controversy, which is the reason why we decided to determine the degree of inclusion of a contrast medium applied to the myocardium. Materials and methods: This was an experimental study using adult pig hearts, distributed randomly into two groups, with two subgroups each. Group I (n = 6): pig hearts injected with a contrast medium through a simultaneous application with multiple syringes («sowing» pattern) Group II (n = 6): pig hearts injected with an intramyocardial contrast medium with an only syringe. The hearts were subjected to a radiological study to compare the distribution of the contrast medium. Fisher’s exact test was utilized for the comparison between both groups. The value of p to consider the differences as significant was set below 0.05. Results: A more homogenous distribution of the contrast medium was observed in the radiographies taken after the corresponding injections to Group I than in those taken for Group II, with a value for p = 0.002. Conclusion: The «sowing» injection technique is the procedure of choice for this type of patients.
