Respiratory distress syndrome prediction by thorax X ray in neonates with meconium in the amniotic fluid

ABSTRACT

Objective: To study the utility of the chest X-ray in the diagnosis and prediction of the breathing difficulty in neonates born with meconium-stained amniotic fluid (MSAF) and to know if the X-ray indicates, ort predicts, the respiratory distress syndrome (RDS).
Material and methods: To 219 neonates with MSAF were taken a chest X-rays, which were examined by three pediatritians. The children were integrated in two groups: if they were active at born (group A): n = 201, and another one if they were depressed (group B): n = 18. Data were analyzed according the normality or abnormality of the X-ray and if they had or not the respiratory distress syndrome.
Results: In the A group there were 194 normal chest X-rays and 7 abnormal. In the B group there were 14 normal chest X-rays and 4 abnormal. There were not differences for age gestational, weight, via from birth, sex, trophism and Apgar. The sensibility of the chest X-ray to support the diagnosis of the respiratory distress syndrome was low (50%), but its specificity to reject the syndrome with a normal image was of 97.2%.
Conclusions: The chest X-ray doesn’t indicate that the neonate has the respiratory distress syndrome, but a normal chest X-ray allows, with a wide margin of certainty, to reject this syndrome.
