Predisponent and desencadenant causes of congestive heart failure in 400 cases

ABSTRACT

Has the heart failure causes like: Specific cardiovascular disease as predisponent, and a desencadenant cause from the same disease, acute from no cardiac origin or from structural damage? Methodology: Four hundred patients with heart failure were studied with clinic, electrocardiogram and chest X ray. Some included pulmonary scanner, antistreptolysin, C-reactive protein, blood cultures and arterial gases. Results: Predisponent causes were: rheumatic heart disease (52.5%), ischemic artery disease (23.5%), systemic hypertensive heart disease (9.5%), cardiomyopathy (8.25%) and pulmonary hypertension (6.25%). There were desencadenant causes: from same the cardiac disease in 290 patients 72.5%, from associated pathology 9.25% and non responding ventricular dysfunction 18.25%. In the rheumatic heart disease the causes were: Atrial fibrillation, pulmonary embolism, rheumatic fever and infective endocarditis. In ischemic heart artery disease we found acute myocardial infarction, angor pectoris, arrhythmias and left ventricular aneurysm. In systemic hypertension the high blood pressure and cardiac arrhythmias were the acute factors. In cardiomyopathy: cardiac arrhythmias and pulmonary embolism. And in pulmonary hypertension: the acute bronchitis, cardiac arrhythmias and pulmonary distress. From no cardiac origin in 37 patients (9.25%): Pregnancy, digitalis intoxication, pneumoniae and gastroenteritis. Heart failure from cardiac structural damage in 73 patients (18.25%). Conclusion: There are causes predisponent and acute in heart failure.
