Unilateral pulmonary edema; an unusual presentation of systolic dysfunction. A case report

ABSTRACT
Acute unilateral pulmonary edema is a rare manifestation of heart failure, which can be misdiagnosed as lung disease. We present a male patient of 69 years old, with passive smoking cigarettes and type 2 diabetes as assessed by dyspnea, whose initial chest radiograph showed an extensive alveolar infiltrate unilateral in right lung which motivated an extensive investigation, already one month, by pulmonologist, which included laboratory tests, sputum, bronchoscopy, transbronchial biopsy, chest CT and simple skin test, all with negative results or nonspecific. Finally, an echocardiogram showed severe systolic dysfunction, mitral and tricuspid insufficiency both moderate and moderate to severe pulmonary hypertension. With conventional treatment for heart failure the patient showed clinical and radiological gradual improvement in hospital. We discuss their etiology, emphasizing the need for a careful clinical analysis to avoid delays in the institution of proper treatment.REFERENCES
