Post-traumatic pseudocyst of the spleen. Report of one case

ABSTRACT

Objective: To inform the case of a pseudocyst of the spleen associated to trauma in a young man.
Setting: Rural second level health care hospital in San Luis Potosí, SP, Mexico.
Case report: Man of 18 years of age that fell from a 3 m high tree, and went to the hospital after 24 hours. He presented moderate, diffuse abdominal pain; exploration did not reveal characteristic pain points but rather generalized discomfort. Vital signs were: Cardiac frequency of 80/min, arterial pressure of 140/80 mmHg, and respiratory frequency of 40/min. Laboratory tests revealed hemoglobin of 12.3 g/dl, hematocrit of 45, leucocytes of 9900, the rest were normal. The ultrasound study revealed the presence of free liquid in the splenic fossa. During surgery, a pseudocyst of the spleen of 8.5 cm in diameter was found, constituted by dense fibrous tissue. Splenectomy was performed. The histopathological study revealed an internal covering of cuboid cells.
Conclusion: Persistence of pain and the presence of free liquid revealed by the ultrasound are indications for surgical treatment in the presence of a splenic pseudocyst.
