Laparoscopic treatment of benign pathology of the colon

ABSTRACT
Objective: To inform the results obtained with laparoscopic treatment of benign colon pathology.
Setting: Third level health care hospital.
Design: Prospective, longitudinal, observational, nonrandomized, non-comparative study.
Patients and methods: We analyzed the results and complications of 34 consecutive laparoscopic surgeries of the colon performed in non-selected patients between November 1993 and September 1998. Main indications for surgery were: 1) complication of diverticular disease (recurring diverticulitis in 17 patients, perforation in 5, colonic stenosis in 5, and hemorrhage in 1). 2) Severe chronic constipation in 2 patients. 3) Reversion of Hartman procedure in 2 patients. 4) Ileocolic intususception in 1 patient. 5) Sigmoid volvulus in 1 patient. Histological confirmation was obtained for all resection cases.
Results: Eleven patients were men and 23 were women. Their ages ranged from 48 to 87 years with an average of 63. Twenty-six surgeries were programmed and 8 were emergency surgeries. Twenty sigmoidectomies, 12 left assisted hemicolectomies, 2 colostomy closures of Hartman procedures were performed; two cases were resolved without resection. Average surgical time was 120 min (from 90 min to 4 hours). Average hemorrhage was of 300 ml (from 150 to 700 ml). Average hospital stay was of 96 hours (from 48 hours to 7 days. Two patients (6%) presented complications and one died (3%). Follow-up period varied from 30 days to 58 months, with an average of 18 months.
Conclusion: Laparoscopic treatment of benign colon diseases is feasible, safe, and efficient, offering the advantages of minimally invasive surgery.
