Learning curve in laparoscopic surgery: laparoscopic nephrectomy experience in laboratory animal care facility

ABSTRACT

Objective: To evaluate the learning curve for nephrectomy utilizing animal models in a laboratory animal care facility.
Materials and Methods: Two surgeons were prospectively and comparatively evaluated utilizing 20 pigs from the laboratory animal care facility for laparoscopic nephrectomy from March to November 2007. Operative time, hilum dissection and ligation time, bleeding, vascular, intestinal and renal parenchyma injury and injury to other organs were evaluated. Results were analyzed to determine the number of procedures in which it is possible to standardize surgical time and the results found in the other parameters.
Results: Each surgical group operated on 20 kidneys, with the same surgeon operating on both kidneys of each pig. Mean kidney and ureter resection time was 42 minutes with a 22 to 80 min. range. Approximation to the mean ocurred after 13 procedures for Group I and after 15 procedures for Group II. There were 18 renal parenchyma injuries, 5 renal vessel injuries, 1 liver injury, 2 spleen injuries and no injuries to other vascular or intestinal structures. There was no statistically significant difference between groups.
Conclusions: This prospective and comparative study showed a learning curve was obtained around the 13th procedure, demonstrating the usefulness of surgical laparoscopic practice on animals at a laboratory animal care facility.
