Carcinoid tumor of appendix. Case report and literature review

ABSTRACT

Appendiceal carcinoid tumor is a neoplasm that is derived from neuroendocrine cells in the submucosa. It affects individuals younger than 50 years, mostly women, and is usually an incidental finding on abdominal gynecological surgery, less than 10% causing symptoms. We present a case of a 40-year-old female patient, who presented pain in the lower abdomen, fever of 38.5 °C, leukocytosis of 15 500 cells/mm3, the ultrasound revealed cystic-solid mass of 6 x 2,5 cm in the zone of the right annex. Right oophorosalpingectomy and prophylactic appendectomy were performed by laparotomy. The anatomopathological study of the Fallopian tube and ovary demonstrated right abscessed salpingoophoritis without evidence of neoplastic infiltration. Macroscopically, the cecal appendix showed a lusterless, smooth, yellowish serosal surface and a thickened wall with a solid, circumscribed, yellowish, lesion on its distal end which contained mucoid material. The histological report described a carcinoid tumor with a classic infiltrating pattern, invading blood and lymphatic vessels of the serosal layer. Immunohistochemical analysis was performed, which confirmed the diagnosis of carcinoid tumor of the appendix.
