Cholecystectomy and single-port laparoscopic common bile duct exploration

ABSTRACT

The pathology of the bile duct for choledocholithiasis has been solved with different methods: open surgery, laparoscopic surgery or endoscopic procedures. We present an alternative for cholelithiasis associated with choledocholithiasis through a laparoscopic cholecystectomy and exploration of common bile duct with one port. From November 2005 to November 2010, 32 patients were operated with this technique. Were the subject of inclusion, choledocholithiasis diagnosed during intraoperative time. Laparoscope with working channel and long (5 mm x 45 cm) laparoscopic instruments were used. We excluded patients with cancer, cirrhosis, and general contraindications for surgery. We analyzed gender, age, BMI, operative time, hospital stay and feasibility of the procedure. Of the 32 patients operated, 25 were women and 7 men, aged 24 to 85 years, an average BMI of 28. Operative Time with an average of 130 minutes. Hospital stay 48 h. The procedure was possible in 28 patients (87.5%) and 4 patients (12.5%) had conversion to open surgery due to technical difficulties. There was no mortality. You can perform this technique as an alternative to current procedures. It requires more cases and further studies to give strength to the procedure.
