Laparoscopic cholecystectomy with one visible subxiphoid single port of 5 mm. Modifications to the technique

ABSTRACT

We present our experience in laparoscopic cholecystectomy performed with one subxiphoid visible port of 5 mm and modifications to traction rein of this technique. Prospectively 750 patients were analyzed on May 1999 to May 2009, operated in a private hospital of laparoscopic cholecystectomy with one subxiphoid visible port. We excluded patients with gallbladder cancer and choledocholithiasis. We analyzed age, surgical time and technical feasibility. Technique modifications include the introduction through the subxiphoid port of rein provided with an end stoppage at one extremity of the suture. The technique had a viability of 96 with 4% conversion to open surgery or conventional laparoscopy. Patients with mean age 57 years old and average operating time of 65 minutes. The amendment to rein facilitated the development of the technique, to reduce a surgical step, allowing more precision selecting the point of exteriorization of the needle and easy removal of the gallbladder. The feasibility is high and the learning curve is short for this procedure. The modified rein can be applied in conventional laparoscopic surgery, single port surgery and natural orifices transluminal surgery.
