ABSTRACT

Supernumerary teeth are developmental disorders that can appear in any maxillary region, though their presence in the mandible is rare. Whilst their etiology is not well-known, it has been postulated that they are the result of a “doubling” of the dental lamina. A study of pedigrees suggests a possible genetic influence, particularly in the case of the mesiodens. Mesiodens can cause several complications, such as the crowding, delayed eruption or displacement of dental organs. Diagnosis is carried out clinically and radiographically. In this paper, we describe the clinical management of a male patient, 4 years 6 months old with three mesiodens, who was treated at a pediatric dental clinic at the School of Dentistry of the Universidad Autónoma del Estado de México. Intraoral examination revealed a conical tooth eruption, and x-ray analysis showed the presence of an inverted dental organ in the midline. Due to the observed displacement of permanent teeth, it was decided that the erupted conical supernumerary would be removed in the conventional manner, and subsequent surgery was scheduled for the inverted mesiodens. Six months later, the control x-ray prior to surgery revealed the formation of a new mesiodens at the site where the conical tooth had been removed. Surgery was performed and it was decided that the patient would be kept under observation for the next few months.
