ABSTRACT
The concept of Minimal Intervention (MI) is well
described in the literature and summarizes the clinical
logic of prevention strategies related to the
management of dental caries. The MI Consultative
Boards of GC Europe, the European group of
academics and general dental physicians, seek to
develop evidence-based patient-centered MI
treatments that can be used in everyday dental practice.
Dental caries is a degenerative process that affects the
hard tissues of the teeth, enamel and dentin. Caries is
caused by imbalances in the oral biofilm, which can be
caused by variations in the minerals, proteins or
microorganisms that form part of it. Dental caries can
be reversible in its early stages, when it appears
clinically as a white spot; or irreversible, when
cavitation is already present. It is in the reversible white
spot stage when remineralizing agents are of great
importance.
It is precisely in the search to find components that
intercept the evolution of dental caries that products
such as fluorine and casein phosphopeptide-amorphous
calcium phosphate (CPP-ACP) have been discovered.
Fluorides serve to stabilize the cyclical process of
demineralization and remineralization. Fluoride acts
when the pH of saliva is below 5; the fluorine atoms
replace the OH ion of apatite creating fluorapatite, a
component that is much more resistant to low pH and
therefore to demineralization.
In the case of casein, recent studies have shown that the
nanocomponents of casein phosphopeptide-amorphous
calcium phosphate (CCP-ACP) help to prevent
demineralization and promote remineralization of the
enamel. This serves to stabilize the calcium ions and
make these soluble so that the teeth can be remineralized.
Furthermore, the CCP-ACP complex may also have a
synergic effect with the fluoride ions.
