Ventilator associated pneumonia in pediatrics, NAVM, pneumonia, bronchial secretions cultivation.

ABSTRACT

Introduction. The nosocomial pneumonia represents the second most frequent cause of such infections, especially when they are associated with the use of mechanical ventilation, which increases the morbidity and mortality of patients affected as well as cost and length of hospital stay. The international literature mentions that gram negative germs represent the normal flora most frequently found in cultures of bronchial secretions in patients with nosocomial pneumonia associated with mechanical ventilation (NAVM). Objective. To determine the incidence of ventilator associated pneumonia in the Pediatric Critical Care Unit. Material and methods. A prospective study, longitudinal, transverse, were included pediatric patients admitted to the Pediatric Critical Care Unit who were subjected to mechanical ventilation discarding those who have income at or immunosuppression pneumonia is growing volume of bronchial secretions, 48 hours after maintained in ventilation and monitor its evolution, using the criteria of Johanson and the outcome of cultivation of bronchial secretions are determined the presence of pneumonia associated with ventilation. Results. We included 35 patients in the study, nine of them presented NAVM representing an incidence of 25.7% and a cumulative incidence of 34 per 1,000 days of ventilation; the bacteria most frequently was isolated Pseudomonas aeruginosa. Discussion. The literature at the international level reported an incidence of pneumonia associated with mechanical ventilation of 22.3% in the U.S. and 23% in Europe, and an incidence rate of six cases per 1,000 days of ventilation in the United States average, with ranges ranging from 12.8 to 17.6 cases per 1,000 days of mechanical ventilation. In studies conducted in our country is reported an incidence of 16 to 29%. In our hospital we are top of these figures reported by what is necessary to increase control measures to curb this disease.
