On the use of disopyramide for the treatment of muscle cramps

ABSTRACT

Introduction: Muscular cramps are a common clinical problem, generally benign although annoying. Occasionally they can become a serious condition. There are different published treatments, but quinine is accepted as an effective one. Our group published the usefulness of quinidine in a series of patients with neurological conditions. Based on that work, and because quinidine was withdrawed from the Mexican market, we initiated the use of disopiramide on the basis of therapeutical affinity. Objective: To inform the effect of disopiramide on muscular cramps. Methods: Patients with muscular cramps, alone or associated to other medical conditions were included. The usual clinical variables were recorded, emphasizing the absence of cardiac disease. Disopiramide was used at 100 or 200 mg in a single dose at night. Response was measured according to the patient perception of the proportion of reduction, as well as the interval between administration and perception of the effect. Side effects were evaluated too. Results: Twenty two patients were included: 8 males and 14 females, mean age was 67 years (63/70). Most common diagnosis were: deep and/or superficial venous insufficiency, 12, idiopathic, 4, diverse neurological conditions, 6. Dosage was 100 mg in 19. In 86.3% there was clinical improvement, of 100% in 63%, >50% in 22.7%. One worsened. Average interval for the effect was 2 days and it was maintained all over the treatment. No side effects attributable to the drug were reported. Conclusion: Disopiramide is an effective, safe and affordable treatment for muscular cramps.
