Laparoscopic manegement of gastric volvulus secondary to paraesophageal hernia. Report of a case and review of literature

ABSTRACT
Introduction: Gastric volvulus can be classified according to etiology as primary or secondary, to anatomy as organoaxial or mesenteroaxial, and to onset as acute or chronic. The majority of cases are secondary and organoaxial, usually for a paraesphageal hiatal hernia.
Objective: Presentation of a case and review of literature.
Case: A case of an organoaxial, intrathoracic and chronic gatric volvulus secondary to paraesphageal hiatal hernia is described. The patient is treated by laparoscopy performing a Nissen fundoplication and hiatus repair with polytetrafluorethylene mesh reinforcement.
Conclusion: Management of secondary gastric volvulus acute or chronic is always with surgery and the choice of surgical procedure for treatment should be done according to etiology. The laparoscopic approach is safe with or without gastropexy always correcting the underlying mechanisms that cause castric volvulus. In those cases due to paraesphageal hiatal hernia it is recommended to perform a fundoplication, resection of the hernia sac and mesh reinforcement.
