Gallbladder cancer versus pyocholecyst clinical challenge

ABSTRACT
Background: Acute cholecystitis is one of the most common causes for admission to surgical service; the empyema of the gallbladder is a major complication, especially in elderly patients. The election treatment is laparoscopic. Material and methods: Is a 54 male admitted to emergency room with abdominal pain of three weeks, intermittent epigastric and right upper quadrant, accompanied by loss of 9 kg. On physical examination: conjuntival jaundice and diffuse abdominal pain, no fever. Laboratories reveal obstructive jaundice with elevated liver enzymes, ultrasound and magnetic resonance with heterogeneous gallbladder, thickening wall and enhancing pericholecyst suggest a neoplasic etiology. Diagnostic laparoscopy is performed, finding an empyema of the gallbladder, drilled and sealed, laparoscopic cholecystectomy was performed, with drainage and through washing of cavity. The patient goes into general surgical floor with a favorable outcome. Conclusion: Acute cholecystitis complicated even today can be a diagnostic challenge.REFERENCES
