Incidence of duodenal diverticulum in endoscopic cholangiography

ABSTRACT

Objective:The purpose of this work is to know the incidence of duodenal diverticulum in patients scheduled for endoscopy and the morbidity and mortality associated with retrograde endoscopic cholangiography.
Design:This is a prospective, transversal, observational, descriptive study.
Setting:Third level heath care hospital.
Patients and methods:From March 1, 2001 to march 1, 2004, 103 patients were studied through endoscopy and retrograde cholangiography. Presence or absence of duodenal diverticulum, type of diverticulum, feasibility of ampulla cannulation, morbidity (mainly duodenal perforation), and mortality were all investigated.
Results:Mean age was 57 years, ranging from 32 to 90 years. Sixty-four patients were women, and 39 were men. Twelve out of the 103 patients studied had duodenal diverticulum for an incidence of 11.6%, the papilla was found inside the diverticulum in 5 of these patients. Cannulation was possible only in one case. There was no associated morbidity and mortality in this series. Choledocholithiasis occurred in 9 of the 12 cases presenting diverticulum (75%). A tumor inside the diverticulum was found in one case.
Conclusion:Incidence of duodenal diverticulum in our hospitals is high (11.6%), and is consistent with the occurrence described in the literature. Papilla was found to be intradiverticular in 41.6% of the cases. Incidence of duodenal diverticulum increases with age. There was no morbidity and mortality in this series.
