Alternative technique to handle and incidental injury of an abdominal wall artery caused by an infra-xiphoid laparoscopic trocar

ABSTRACT

Objective:To describe a solution alternative to reach hemostasis of an epigastric artery injury induced by the introduction of an infra-xiphoid port.
Design:Prospective, longitudinal, observational study.
Material and method:Prospective, longitudinal, observational study of 100 laparoscopic cholecystectomies. Direct variables: Bleeding of the wall induced by the introduction of the infra-xiphoid port and its resolution by means of applying a temporal suture in X with total abdominal wall thickness. Indirect variables: Age, gender, need to extend the infra-xiphoid port, post-operative pain intensity, hematoma or infection of the port-site wound. Statistical analysis was made by means of percentages as a summary measure for qualitative variables.
Results:From the patients, 79 were women and 21 were men; average age of 36 years. Five patients presented bleeding, no bleeding was observed in any of the cases after applying the suture in X of total abdominal wall thickness; two of these cases presented a small hematoma that did not require any further treatment. None of the cases required extension of the incision, and no surgical wound infection developed.
Conclusion:The use of total thickness suture is a simple and low-cost method to control bleeding induced by the incidental injury of the superior epigastric artery.
