Monitoring of exhalation of carbon dioxide in the process of dying

ABSTRACT

Measuring the exhaled carbon dioxide (PetCO2) is a useful tool in the intensive care units as a method of noninvasive monitoring attempts to estimate the partial pressure of carbon dioxide in arterial (PaCO2) and thus can be obtained information of ventilation, diffusion and perfusion. The main objective of this study was to monitor PetCO2 in the process of dying and secondarily determine a theory to justify this phenomenon.
Material and methods: Seven patients admitted to the Intensive Care Units who had orders not to resuscitation and were in the process of dying, for nine minutes was recorded PetCO2 behavior while on mechanical ventilation.
Results: We obtained an average of PetCO2 at the beginning of 14 ± 4.5 and at the end (four minutes after missed the gradient of pressure between the MAP and CVP) of 5.4 ± 1.1. In the nine minutes monitoring the record of PetCO2 never reached zero. Found an r2 of 0.49 between the gradient of pressure (MAP and CVP) and PetCO2.
Conclusions: In this study we monitored PetCO2 in the process of dying, finding persistence on the record during the nine minutes monitoring was maintained. Further studies are needed to understand what is the duration and what are the causes for this phenomenon.
