Factors Influencing the Result of Patients with Glottic Cancer in Early Stage Submitted to Transoral Microsurgery with Laser CO2

ABSTRACT

Background: CO2 laser transoral microsurgery has been used as a treatment in patients with glottis cancer in early stages.
Objective: To know the risk factors of recurrence of patients with glottis cancer in early stages postoperated with CO2 laser transoral microsurgery.
Material and method: A retrospective study was done in a third level hospitalary center. A cohort of 55 patients was analyzed from October 2003 to June 2010 with a follow-up of, at least, eight months. Descriptive analysis and of correlation of qualitative and quantitative variables were done by contingence coefficient, phi correlation and Spearman R, respectively. By log rank survival was analyzed comparing patients submitted to CO2 laser transoral microsurgery as initial treatment vs those who were not submitted to this treatment.
Results: Smoking, alcoholism and diabetes mellitus were significant risk factors of recurrence of laryngeal cancer. Also, the management with CO2 laser transoral microsurgery as first modality of treatment conferred adequate survival, patients submitted to other initial modality of treatment had a higher incidence of complications.
Conclusions: Smoking, alcoholism and diabetes mellitus were significant risk factors of recurrence of laryngeal cancer. The safety and efficiency of CO2 laser transoral microsurgery were demonstrated as treatment of glottis cancer in early stages.
