Key points in the endoscopic control of laparoscopic Heller miotomy

ABSTRACT

Achalasia is a uncomun primary disorder of the esophagus caracterized by a loss of peristalsis in the esophagus and a failure of lower esophageal sphincter relaxation. The diagnosis can be oriented by contrasted radiological studies, endoscopy or a motilitity study, wich today is considered the gold standard for diagnosis. The treatment include pneumatic dilatations, botulinum toxin injection in the gastroesphageal junction by endoscopy, pharmacological therapy for those patient with surgical high risk, peroral endoscopic myotomy (POEM) or laparoscopic myotomy with or without fundoplication, wich today is considered the gold standard treatment. The endoscopic control during the laparoscopic procedure improves safety and results by giving a precisely view of anatomic references.
