Characterization of obstetrical patients admitted in intensive care by hypertensive disorders related to pregnancy

ABSTRACT

Introduction: Hypertensive disorders during pregnancy include a variety of conditions. Objective: To characterize obstetric patients with hypertensive disorders. Method: We performed a retrospective study of patients admitted to the intensive care unit of the Docent General Hospital: "Dr. Enrique Cabrera", between October 2011-2012, with the diagnosis of hypertensive disease of pregnancy. We reviewed the medical records of 33 patients with this entity. Results: The mean age of the patients was 26 ± 6 years, no predominance of primiparous over multiparous was observed and 45.46% of the patients were diagnosed in the postpartum period. The mean APACHE II score on the first day was 5.84 ± 3.06. The predominant clinical presentation was severe pre eclampsia in 17 patients (51.51%) followed by eclampsia in 12 (36.36%). In six patients systolic pressures were found above 180 mmHg and the diastolic pressures above 110 mmHg in 13. In 29 patients (87.87%) alpha methyl dopa as antihypertensive was prescribed and magnesium sulphate (prophylactic or therapeutically) was used in 27 cases. Conclusions: The main complication was the acid-base balance disorders in 42.42% of the patients. The average stays were 5.21 ± 3.27 days without maternal deaths and only one stillbirth.
