Fabrication of surgical obturator (immediate prosthesis) in a patient with hemimaxillectomy for epidermoid carcinoma

ABSTRACT
A 54 years old woman attended the Dental School at the Universidad de la Salle Bajío in October of 1999, she was referred from the Hospital General Regional to make her a surgical obturator. She was diagnosed with epidermoid carcinoma and she was to be submitted for hemimaxillectomy in November of 1999. The patient was sent to the postgraduate studies division of prosthodontics and implantology at the same university. Objective: Two techniques were performed for the fabrication of the surgical obturator and determine which one of them provides the best characteristics of stability, support, retention and insertion. Laboratory process: Technique 1. The extension area to be resected was determined, the initial cast was duplicated and waxed including ball hooks on the remaining teeth, it was flasked and teeth were acrylized with fast setting acrylic, the rest of the denture was acrylized with normal setting acrylic. Technique 2. At the same time a 0.80 mm acetate base prosthesis was made in a vacuum machine. The remaining teeth extension area included the whole coronal surface and the palate’s maximum extension was preserved. Conclusions: The acetate base technique shower the best results of the two performed techniques, besides to be very light this obturator it presents advantages such as saving time, simple elaboration, adaptation and placing at surgery time. The obturator improves phonation, deglutition, mastication, breathing, esthetical aspect and psychological effect.REFERENCES
