Total parathyroidectomy with autotransplant in the treatment of secondary hyperparathyroidism secondary in end stage renal disease

ABSTRACT

Introduction. Secondary hyperparathyroidism is known as a common complication in patients with end stage renal disease presenting itself with hyperplasic parathyroid glands as a blood calcium regulator mechanism.
Materials and Methods. An observational, descriptive and retrospective study was conducted from January 2013 to march 2014, both gender patients are included, ages between 0 and 17 years, treated with total parathyroidectomy and autotrasplant as treatment for secondary hyperparathyroidism in end stage renal disease.
Results. Eleven patients were followed, mean age of 10 years, mean presurgical parathyroid hormone of 1225 pg/mL, calcium 9.51mg/dL and phosphorus 5.03mg/dL. A month from surgery median PTH descended to 202.75 pg/mL, calcium to 7.96mg/dL and phosphorus to 3.29mg/dL. PTH median descend levels were statistically significant (p ‹ 0.001) as calcium (p=0.012) and phosphorus (p=0.011).
Discussion. Total parathyroidectomy and autotrasplant is an effective procedure with low complication rates and satisfactory outcomes for secondary hyperparathyroidism in children with end stage renal disease.
