ABSTRACT

Introduction. Arteriovenous fistula are frequently the result of a vascular trauma complication from a penetrating wound.
Objective. Present a clinical case that was treated 24 years after a vascular trauma occurred.
Material and methods. A 50 years old male patient, who was wounded at the right arm with a firearm projectile, 24 years ago. Diagnosis of an arteriovenous fistula was clinically made and the injury anatomy was detailed with an angiotomography. The injury was then repaired with open surgery.
Results. The patient presented immediate improvement, murmur and thrill disappeared and the injured limb presented volume reduction.
Conclusion. Arteriovenous post-traumatic fistulas are frequently diagnosed at a late stage. Clinical diagnose and its anatomy must be detailed using a Doppler duplex and angiographic studies. Endovascular surgery is the ideal method to correct this pathology. Conventional surgery remains as an excellent alternative.
