ABSTRACT
We present a rare case of gluteal necrosis after kidney transplant surgery and its evolution to resolution. Ligation of the internal iliac artery (right or left) due to end-to-end anastomosis in renal transplantation has been used since the beginning of transplantation and the flow in the distal segment is maintained due to the extensive collateral circulation, thereby maintaining adequate vascularity to muscles and pelvic organs. Complications are very rare, but possible. The necrosis of the gluteal region has been reported as a complication related to the radical gynecological surgery, but never reported when ligation is performed distal to the posterior trunk as in this case. The gluteal necrosis in renal transplantation is a serious complication not previously reported. We ignore the cause, but it was probably secondary to an anatomical abnormality that conditioned acute ischemia of the gluteal region. Given these complications, aggressive multidisciplinary intervention is necessary to prevent serious consequences.REFERENCES
