ABSTRACT

The clinical case of a 33 years old attended the Highly Specialized Regional Hospital of Oaxaca diagnosed with Fever of Unknown Origin is presented. The clinical picture began in October 2012, with initial diagnosis of rheumatoid arthritis, for which he received medical treatment for gastric intolerance left. Three months after it was submitted to the same hospital for weight loss of 11 kilograms, intermittent fever of 39 oC and aortic murmur; which he ruled out the diagnosis of rheumatoid arthritis. The transthoracic echocardiography showed aortic insufficiency, Chiari network bicuspid aorta valve and attached filiform injury. It was decided to perform surgery with valve replacement, achieving clinical improvement. In the case analysis the role of new imaging tools that have contributed to the modification of the concept of Fever of Unknown Origin stands.
