Gasless video assisted thyroidectomy

ABSTRACT

Introduction: Neck surgery is one of the most recent applications for the approach of endoscopic or video assisted surgery. Initially used for the treatment of parathyroid gland pathology, the endoscopic approach to the neck can be done under CO2 insufflation or by external retraction in order to create the working space. World experience in endoscopic or video assisted thyroid surgery is limited. We present the case of a patient with a thyroid nodule resected by video assisted endoscopic neck surgery. Case report: A 42 year old female presented a cold nodule 3 cm in diameter localized in the left thyroid lobe found one year before. Fine needle biopsy found a follicular neoplasm. A left endoscopic video assisted hemithyroidectomy was performed under a 28 mm transverse cervicotomy, a 5 mm 30o laparoscope and small fine instruments were utilized for dissection of the anatomic structures. The patient had a satisfactory evolution and was discharged on the first postoperative day. Conclusions: Endoscopic video assisted technique for thyroidectomy can be done safely and effectively with a better cosmetic result. Nevertheless more experience is needed in order to decide if this technique should be routinely used in small thyroid nodules.
