Report of 604 cases of laparoscopic cholecystectomy performed by the same surgical team

ABSTRACT
The first cholecystectomy was performed in 1882 by Carl Langenbuch, maintaining itself for almost 110 years as the elective procedure for the gallbladder disease. In the final years of the 80´s, the laparoscopic procedure took rapidly a place in the management of the gallbladder disease.
Methods: 604 cases of laparoscopic cholecystectomy performed by the same surgical team between September 1990 and February 2001 were analyzed retrospectively.
Findings, evolution, complications, and days of hospital stay were evaluated in each patient.
Results: From this 604 patients, 463 (76.6%) had chronic lithiasic cholecystitis and 141 (23.4%) acute cholecystitis.
Ultrasound of the liver and bile duct was performed as a diagnostic method in 100% of the patients.
367 were women (60.7%) and 237 men (39.3%), ages fluctuating between 8 and 89 years, (average 47.4 years).
All procedures were planned as laparoscopic cholecystectomy, requiring conversion in two cases.
Complications were divided in:
a) Major: Lesion of the bile duct in 1 case (0.16%) and biliary peritonitis due to dehiscence of the cystic duct in 1 case (0.16%).
b) Minor: Hematoma of the surgical wound in 10 cases (1.6%), seromas of the surgical wound in 17 cases (2.6%) and acute retention of urine in 16 cases (2.36%).
Surgical time was 38 minutes ± 19 minutes.
Trans-surgical cholangiography was performed in 299 patients (49.6%), without complications during the procedure.
Hospital stay was 35 ± 16 hours.
