Ischemic EVC and pulmonary thrombo-embolism associated to permeable foramen ovale: Case report

ABSTRACT

A 53-year-old woman admitted to the emergency room after sudden loss of state of alert Glasgow 3. She had a clinical history of saphenectomy two days before admission. Upon arrival, endotracheal intubation was performed and vasopressors were administered due to persistent hypotension. After a cerebral CT scan, cerebral ischemic stroke was diagnosed, and the patient was submitted to endovascular therapy. During the procedure she presents persistent desaturation despite O2 supply at 100%. A pulmonary CT scan showed pulmonary thromboembolism and a transesophageal echocardiogram was performed which demonstrated patent foramen ovale. Six hours later stem reflexes were lost, and a cranial CT scan confirmed hemorrhagic brain conversion. Brain dead was diagnosed a day later and the patient was considered for the transplant protocol surgery where kidneys, liver, bone and corneas were donated.
