Laparoscopic cholecystectomy in patients with heart transplantation and chronic immunosuppression. Case report

ABSTRACT

The results of the laparoscopic experience during the last nine years in a third level public hospital like the Hospital General de México, O.D. are exposed. A revision of the clinical files of all patients that underwent a programed laparoscopic cholecystectomy was performed from January 1, 1993 to December 31, 2001 whose results are being exposed as a descriptive statistic. A total of 7,323 cholecystectomies were performed, of which 3,394 were laparoscopic and the rest open (3,929), with a prevalence of women in a 3:1 ratio; the average age was 52 years. During the first year only 65 laparoscopic cholecystectomies were performed, 525 in 1998, 536 in 1999, 610 in 2000 and 461 in the year 2001. There were 160 conversions for a 4.71% and seven cases with lesion of the extrahepatic biliar duct for a 0.23%. The main causes for conversion were: difficulty to identify the anatomy, unidentified bleeding and the presence of multiple adherences; 179 cases were presented with some type of complication , however, only 94 required conversion. Conclusions: Laparoscopic cholecystectomy is a safe procedure in experienced hands, however nobody is exempted of complicatons that require conversion to the open form. The surgeon must be qualified to solve this complications and have the knowledge to decide the moment of conversion.
