Laparoscopic appendectomy. The preferred approach for all cases?

ABSTRACT

Background: Laparoscopic appendectomy is a valuable alternative, however there are doubts about its safety in complicated appendicitis. Objective: Present the results with laparoscopic appendectomy as the choice for all cases of appendicitis in a cohort. Material and methods: Retrospective study of patients with acute appendicitis who underwent laparoscopic appendectomy. According to the histopathological report, cases were divided in group A, (edematous and suppurative appendicitis) and B (gangrenous or perforated), describing surgical technique, age, sex, evolution and surgical time, antibiotics, hospital stay, complications and conversions. Results: 123 cases were collected, 56 men, 67 women. 81.3% were group A, and 18.7% B. Mean evolution time was 23.6 and 34.10 hours, in group A and B. Mean operating time was 61.44 minutes and 99.35 minutes in group A and B respectively. Average hospital stay was 2.7 and 3.7 days, in group A and B respectively. No mortality, conversion, or reoperation was seen in group A or B. Four patients had infection at the umbilical port site. Conclusions: In this series even with complicated appendicitis there were no conversions, reinterventions, or major complications. These results with other published suggest that laparoscopic appendectomy may be the first choice even in complicated cases of appendicitis.
