Tentorial dural arteriovenous malformation manifesting as trigeminal neuralgia treated by endovascular therapy: Case report

ABSTRACT

Objective: To describe a rare case of trigeminal neuralgia (TN) secondary a dural tentorial arteriovenous fistula (DAVF).
Case report: We describe a case of a 70 year-old male with diagnosis of TN, an MRI showed a hypointense signal lesion with a hyperintense core in the left perimesencephalic cistern suggesting the occurrence of a vascular structure. The diagnosis of TN due to vascular malformation was established. Then a cerebral angiography showed a Dural Arteriovenous Fistula dependent of the left carotid system. He was treated by endovascular therapy. The following day after the procedure was completed the patient showed marked improvement of his symptoms, without presenting pain recurrence and he was hospital discharged 4 days after admission. The patient remains asymptomatic 6 months after the procedure.
Conclusions: TN secondary to DAVF is an extremely rare condition. Our patient represents the 15 reported case in the world literature. Although several therapeutic approaches have been described, endovascular therapy is more constantly employed in these cases. In the patient described here endovascular treatment produced pain remission with no recurrence. Though, in TN cases due to DAVF the optimum treatment remains unclear.
