Gastric leak after laparoscopic sleeve gastrectomy. Classification, diagnosis and treatment

ABSTRACT

Introduction: Laparoscopic sleeve gastrectomy is a relatively simple and effective procedure for weight loss; nevertheless, it implies more serious and harder to treat complications when compared to a gastric bypass surgery; which includes gastric leak. Objective: Our purpose in this paper was to analyze the available literature on regards of bariatric surgery to establish simple and practical guidelines for the classification, diagnosis and treatment of gastric leak after a laparoscopic gastric sleeve. Material and methods: A PubMed search for medical articles using the keywords «gastric leak and sleeve gastrectomy» was carried out within the period from 2009 to 2016; a total of 245 papers was found; of them, only 18 complied with the criteria to be included in this study. Conclusion: This review is a simple and practical guide for the early diagnosis and proper management of gastric leak. The key to avoiding this dreaded complication is the careful and appropriate selection of patients, the experience of the surgeon, a gentle tissue dissection, a proper selection of the technique and surgical team, as well as avoiding medial or distal tubular stomach stenosis.
