Single port vertical sleeve gastrectomy. Surgical procedure and case report

ABSTRACT

Background: The vertical sleeve gastrectomy is a surgical treatment for obesity known as a safe and effective method for permanent weight loss. The standard endoscopic technique requires four to seven trocars; however, in some centers worldwide this procedure has been performed through an umbilical single port with improved aesthetic results and without increasing the risk of complications. Most of these reports come from large concentration hospitals in European countries. Special surgical instruments are required in addition to the umbilical multiport device, such as curved clamps and flexible lenses. There is no report available about this approach in our population. Case report: This is the first case of vertical sleeve gastrectomy using an umbilical single port performed at our hospital. A 20-year-old female with a BMI of 36 kg/m2 underwent this surgical procedure in our center once the established pre-op protocol was accomplished; we used the umbilical multiport system SILS (Covidien®). The total operating time was 135 minutes, and the blood loss, 30 cm3. An Echelon Flex® (Ethicon) stapler was used, as well as conventional endoscopic clamps and lenses. After six months, a weight loss of 35 kg was accomplished with no complications and a practically imperceptible umbilical scar. Conclusions: It is feasible to perform a gastric sleeve in our setting with conventional endoscopic instruments using the umbilical multiport access device.
