Otelo’s syndrome. A case study

ABSTRACT

Introduction: Otelo’s syndrome is a psychiatric disorder of delirious idea, which is typically caused by jealous.
Objective: Analyze the diagnostic guidelines in Otelo’s syndrome taking a typical case as a starting point.
Method: A review of updated bibliography up to 2016 about diagnostic guidelines in Otelo’s syndrome, and the advances on its etiology and treatment was conducted.
Results: A case of a 51 year-old man, that is a father and a prestigious professor, who came to psychiatric consultation with his wife, after making a public scene, is presented. At the doctor’s office he was considered as an oriented patient, well-mannered, having good hygienic habits, and without any disorders in the moving, emotional and cognitive spheres. His wife said he subjected her to interrogation and insults continuously, because of a strong sensation of marital infidelity, that has never been proved. In this occasion, the cause of the quarrel were unfounded jealous again. Things have reached such a point that he attacked his children and wife physically, something that had happened before too. At the end of the monitoring and treatment using risperidone, it was observed clinical improvement ,so the case was considered as a disorder of delirious idea, typically caused by jealous, which is known as Otelo’s syndrome.
Conclusions: Otelo’s syndrome has not changed diagnostic guidelines, including atypical antipsychotics in the usual therapeutics.
