Paracetamol overdose in a newborn: a case report

ABSTRACT

Since the marketing of intravenous acetaminophen, cases of overdose secondary to errors in administration have been reported. We present the case of a term newborn, in whom who incidentally was given 250 mg of intravenous paracetamol in a hospital setting, as part of the management of post-surgical pain. With plasma paracetamol levels over the Rumack-Matthew normogram treatment line, early treatment with N-acetylcysteine was initiated. During hospitalization he did not develop symptoms of liver toxicity. It is assumed that safety and tolerance to paracetamol in neonates is due to the greater sulfation capacity than glucuronidation as well as lower expression of cytochrome P-450 in the immature liver of the newborn.
