Actinomycetoma by Nocardia spp. Report of Eight Cases in Izúcar de Matamoros, Puebla

ABSTRACT
Background: mycetomas are caused by fungi and actinomycetes, called eumycetomas and actinomycetomas. Actinomycetoma affects mainly male farmers in Izúcar de Matamoros, a small city in a Mixteca area, in the State of Puebla, because it has the proper sub-tropical and low socio-economical environment .Objective: to described the sociodemographic, clinical and microbiological characteristics in eight cases of actinomycetoma from Izúcar de Matamoros, in the State of Puebla.
Methodology: We obtained in two years (2015-2017) all the information about the mycetomas from patients from Izúcar de Matamoros, from the archives of the mycological laboratory at the University Hospital in Puebla, and from the Mycologycal Section at Dr. Manuel Gea Gonzalez General Hospital in Mexico City, and from two community dermatological consults in Izúcar de Matamoros. Mycological studies and treatments are reported.
Results: We studied eight cases, two women and six men (75%). Fie farmers, one student, one housewife and one builder. The trunk was most commonly afected (50%), and the lower extremities in 37.5%. Nocardia spp. was isolated in all eight cases and just one case of Nocardia brasiliensis was confirmed by molecular biology. Two cases were hospitalized to treat them with amikacine and imipenem iv, the rest were treated with trimethoprim-sulfamethoxazol.
Conclusions: Izúcar de Matamoros is a region with a very poor social and economical conditions; in a sub-tropical area where the main income is the sugar industry. This study depicts the significant incidence of actinomycetomas caused by Nocardia spp. affecting in 50% the trunk and in 37.5% the lower extremities.
